Lars Schuyesmans is 41 years old and lives in Belgium. It wasn't until he was 39, after a search spanning decades, that he finally received the diagnosis of STXBP1. For his parents, this finally brought relief and clear answers after nearly 40 years of uncertainty.

My son's name is Lars Schuyesmans. He was born on January 30, 1985 (age 41). He is domiciled in Berchem near Antwerp in Belgium, Europe. During the week, he stays in a facility in Zoersel. Weekend stays are divided between mom and dad.

The diagnosis: Lars was almost 40 years old when he was diagnosed with STXBP1. As parents, it gave us a sense of relief. Finally a diagnosis, finally clarity about what was wrong with him.

The story: When Lars was 13 months old, after a whole series of examinations, the pediatrician informed us that he suffered from subcortical brain atrophy. For 13 years, we lived with that conviction. Following a long trip we had planned, and the suspicion that Lars had outgrown the epileptic seizures, we were told, at the age of 14, after a new brain scan, that the diagnosis Lars had received turned out to be incorrect. Lars had/has a clear developmental delay, but the cause of this could not be identified.

The epileptic seizures returned when Lars was 39. Thanks to the facility where Lars resides, we ended up with a neurologist who, coincidentally, is involved in the study on STXBP1 syndrome. Presumably, she recognized the STXBP1 symptoms and asked us if she could start an investigation. Her suspicion proved correct, and Lars finally received a clear diagnosis.

Primary symptoms: Lars's epilepsy evolved from frequent minor seizures (petit mal up to 400/day) during his early years to many seizure-free years. And from the age of 39, to severe seizures (grand mal) that occur mainly at night and are largely under control thanks to medication (Lamictal).

As a baby, Lars never crawled. He pulled himself forward with his arms while sitting or lying on his stomach. At 2.5 years old, he could walk independently. He has little muscle mass in his arms and legs. He feels most secure on a hard surface because his balance is rather unsteady. He will never take the initiative to step or walk a little faster on his own, but if he is encouraged in a gentle way, he will make a considerable effort for a few meters to pick up the pace. Lars is left-handed and eats with a spoon from a deep plate or a plate with a rim. Fine motor skills are ruled out, partly due to the tremor in his hands.

Lars speaks in three-word sentences. He understands simple language well. When he agrees to something, he smiles. If he does not want something, he makes this clear both verbally ('no!') and non-verbally (looking angry and turning his back to you).

Lars usually sleeps through the night well, although he is often awake very early and full of energy to start the new day.

Current focus: Lars has continued to function at approximately the same level for many years. The search for the right medication for his epilepsy previously had a negative impact on his behavior and functioning (apathetic, listless, pulling funny faces,...) but fortunately with a positive result. Lars is his old and familiar self again.

Personality in 3 words: Cheerful - empathetic (senses emotions) - strong adaptability

Favorite things: Music is indispensable for Lars. His familiar songs (especially children's music) give him a sense of safety and calm that he desperately needs.

Because of his 3 homes (the facility, with dad, and with mom), he attaches great importance to knowing exactly where he will spend the night. Therefore, every bed has his stuffed animal. A (grandma) bear in the sunflower, a monkey with dad + a frog in the camper, and a llama with mom. Lars knows very well which animal sleeps in which bed. It gives him clarity and peace.

Uncle Lars is crazy about his nephews and niece. They get along very well. To them, Lars is completely part of the family.

At the facility, Lars has the task of helping to care for the animals at the animal park. He really enjoys this. He feels most at ease with the quiet animals, such as the rabbits and the alpacas. He is also very familiar with the goats. He prefers to keep a safe distance from the more playful animals.

His superpower: His cheerfulness, his infectiousness, his positive attitude, his focus on contact, a warm mood setter.

What advice or hopeful message would you like to give to a European family that has just been diagnosed with STXBP1 syndrome?

Try to let go of your fear of your child being different.

Caring for your child demands a lot from you as a parent, but you receive a great deal of love, warmth, and humor in return, creating a kind of balance.

It is important for your child to feel a continuous warm connection with their caregivers so that they have the reassurance that they are heard or seen when they need to. Good eye contact is very valuable in communicating with your child.

De naam van mijn zoon is Lars Schuyesmans. Hij werd geboren op 30 januari 1985 (41j). Hij is gedomiciliëerd in Berchem bij Antwerpen in België, Europa. Tijdens de week verblijft hij in een voorziening in Zoersel. De weekendverblijven worden verdeeld tussen mama en papa.

De diagnose:

Lars was bijna 40 jaar oud toen hij de diagnose STXBP1 kreeg. Het gaf ons als ouders een gevoel van opluchting. Eindelijk een diagnose, eindelijk duidelijkheid wat er aan de hand is met hem.

Het verhaal:

Toen Lars 13 maanden oud was, werd ons, na een hele reeks onderzoeken, door de kinderarts meegedeeld dat hij aan een subcorticale hersenatrofie leed. 13 jaar lang leefden we in die overtuiging. Naar aanleiding van een lange reis die we gepland hadden, en het vermoeden  dat Lars de epileptische aanvallen ontgroeid was, werd ons, op zijn 14de, na een nieuwe hersenscan verteld dat de diagnose die Lars had, niet bleek te kloppen. Lars had/heeft een duidelijke ontwikkelingsachterstand, maar de oorzaak hiervan was niet aantoonbaar.

De epileptische aanvallen kwamen terug toen Lars 39 was. Dankzij de voorziening waar Lars verblijft, kwamen we terecht bij een neurologe die, als bij toeval, meewerkt aan de studie rond het STXBP1-syndroom. Vermoedelijk herkende zij de STXBP1-symptomen en vroeg ons om een onderzoek te mogen opstarten. Haar vermoeden bleek te kloppen en Lars kreeg eindelijk een duidelijke diagnose.

De epilepsie bij Lars is geëvolueerd van veelvuldige kleine aanvallen (petit mal tot 400/dag) tijdens zijn jonge jaren naar vele aanvalsvrije jaren. En vanaf zijn 39ste naar zware aanvallen (grand mal) die zich vooral 's nachts voordoen en die dankzij medicatie (Lamictal) grotendeels onder controle zijn.

Lars heeft als baby nooit gekropen. Hij trok zich met zijn armpjes al zittend of buikliggend vooruit. Op 2,5 jaar kon hij zelfstandig stappen. Hij heeft weinig spiermassa in zijn armen en benen. Het zekerst voelt hij zich op een verharde ondergrond omdat zijn evenwicht eerder wankel is. Hij zal nooit zelf het initiatief nemen om wat sneller te stappen of te lopen, maar als hij op een fijne manier aangespoord wordt, zal hij een aantal metertjes flink zijn best doen om er wat meer vaart achter te zetten. Lars is linkshandig en eet met een lepel uit een diep bord of een bord met bordrand. Fijnere motoriek is uitgesloten o.a. vanwege de tremor in zijn handen.

Lars spreekt met 3-woordzinnen. Hij begrijpt eenvoudige taal goed. Als hij toestemt met iets, lacht hij. Als hij iets niet wil, laat hij dat zowel verbaal ('nee!') als non-verbaal (boos kijken en z'n rug naar je toedraaien) duidelijk blijken.

Lars slaapt meestal goed door, al is hij veelal erg vroeg wakker en vol energie om de nieuwe dag te starten.

Huidige focus:

Lars blijft al vele jaren ongeveer op hetzelfde niveau functioneren.  De zoektocht naar de juiste medicatie voor de epilepsie, heeft eerder negatieve invloed gehad op zijn gedrag en zijn functioneren (apatisch, futloos, gekke bekken trekken,...) maar gelukkig met positief resultaat. Lars is weer zijn oude en vertrouwde zelve.

Persoonlijkheid in 3 woorden:

Blijgezind - empathisch (voelt gevoelens aan) - sterk aanpassingsvermogen

Favoriete dingen:

Muziek is onmisbaar voor Lars. Zijn voor hem vertrouwde nummers (vooral kindermuziek)geven hem een gevoel van veiligheid en rust waar hij veel nood aan heeft.

Vanwege zijn 3 thuizen (de voorziening, bij papa en bij mama), hecht hij veel belang aan goed te weten waar hij de nacht zal doorbrengen. Elk bed heeft daarom zijn knuffeldier.  Een (oma)beer in de zonnebloem, een aap bij papa + een kikker in de camper, en een lama bij mama. Lars weet heel goed welk diertje in welk bed slaapt. Het geeft hem duidelijkheid en rust.

Nonkel Lars is verzot op zijn neefjes en nichtje. Ze gaan erg vertrouwd met elkaar om. Voor hen hoort Lars er helemaal bij.

Lars heeft in de voorziening de taak om de dieren van het dierenpark mee te helpen verzorgen. Hij vindt dit heel fijn. Hij voelt zich het meest op zijn gemak bij de rustige diertjes, zoals de konijnen en de alpaca's. Ook de geitjes zijn hem goed vertrouwd. Bij de meer speelse diertjes blijft hij het liefst op veilige afstand.

Hun superkracht:

Zijn goedlachsheid, zijn aanstekelijkheid, zijn positieve uitstraling, zijn gerichtheid op het contact, een warme sfeermaker.

Welk advies of hoopvolle boodschap wil je geven aan een Europees gezin dat net gediagnostiseerd werd met het STXBP1-syndroom?

Probeer je angst los te laten voor het anders zijn van je kind.

De zorg voor je kind vraagt veel van jou als ouder, maar je krijgt er heel wat liefde, warmte en ook humor voor in de plaats waardoor er een soort evenwicht ontstaat.

Het is belangrijke voor je kind dat het een continue warme verbinding kan voelen met z'n verzorgers zodat hij/zij de gerustheid heeft dat hij/zij gehoord of gezien wordt als het daar behoefte toe heeft. Goed oogcontact is erg waardevol in de communicatie met je kind.
